Not all; only boats big enough to carry them. Figure that any boat big enough to sleep aboard is going to have some kind of sanitary system, which on a boat is normally called the "head".

For small craft like day sailers, rowboats and canoes, you either hold it till you get back to shore or you "go" over the side.
