The 5th dungeon is the dungeons on the moon. To get to them, you have to complete the 4 regular dungeons, go to the top of the Clock Tower on the night of the final day, and play the Oath to Order. There, you have to give masks to the children to access the dungeons. If you give them the 20 masks other than the Deku, Goron, and Zora Masks, you'll get the Fierce Deity Mask.
