No you can play operation anchorage whenever you want but only once. You can also play the Pitt anytime in the game and you can come and go as you please after you beat it. The only add-on you have to beat the game to play is Broken Steel.

No you can't still play O:A if they're hostile. Try sleeping for about 36-48 in game hours. Generally that cancels out previous hostilities.

Unfortunately since it's so unique (a pistol cut down from a rifle), there isn't a regular version to use for repairs. Your only options are to use a repair kit or pay certain individuals in the wasteland such as Freeside's Mike & Ralph's

In my opinion Fallout New Vegas is. In Fallout NV it is easier to make caps so you can buy better wepons and ammo. In Fallout 3 It's hard to make a lot of caps in the begining making it a slow start. But is you want to test your skills play Fallout 3.
