Xbox live is needed to get DLC. However you do not have to pay for the gold membership. Set up a silver xbox live account, which is free, and you will be able to access the marketplace and get game downloads.

If you have xbox silver you are not aloud to play online, you would have to pay for gold membership to play online but if you dont want gold you can only play non-online things like single player etc.
Monthly membership means paying monthly to continue a membership. For example the monthly membership fee of my Gym is 35$ - that means I must pay 35$ a month in order to keep my membership at my Gym. Another example is that I need to pay 15$ a month to keep on playing World of Warcraft because the monthly membership cost is 15$. Monthly Membership is NOT a one time payment - it is a payment that you must pay every month to keep the product. So if I were to buy an xbox360 for 499$ then that is considered a one time payment since I only need to pay for the xbox360 once. But to get Xbox live on my xbox360, I need to pay 5$ every single month for as long as I have xbox live. So If I had xbox live for 1 year I need to pay 5$ for how many months there are in a year - which is 12. 5x12 =60$, So I pay 60$ a year just to keep xbox live. If I were to stop paying monthly membership for Xbox live I would no longer be able to play xbox live but I also do not need to pay the 5$ a month.
