If you have got a new passport within the last year, then no. But beyond one year, yes.
Your passport is equivalent to your ID card just on an international level. Issued by the government, it is your identification for when you travel and, more importantly, it is also proof of your citizenship. Just as you would go and change your name on your driver's license, credit cards, or other ID cards, you should also change your name on your passport. If your passport has over 1 year of validity and it is within one year of your date of name change, they will just add an amendment to the back of your current passport. The amendment is free of charge unless you require your passport back early. If it has been over one year since your name change you will have to renew your passport and pay all applicable fees. Required forms or any questions can be found in the link provided below.

Yes. That said, I'm not a lawyer so I'd better explain my reasoning. As I understand it, your legal name is that by which you are known. Not necessarily the name on your passport or birth certificate. So, you are at liberty to call yourself anything you like as long as it is not for any criminal intent. If you did not changed your passport when you got married then there is very little to stop you simply using your birth name again. If you did change your passport then you'll have to contact the passport office. I suspect that a Deed Poll might suffice, but I'm only guessing.
Pay your child support.
Go to the post office for the forms for a passport, get the passport picture, pay the fee, and in about 4-6 weeks your passport will arrive. If you need a visa you send your passport to a service with the fees and they will get the visa stamped on the passport. I needed a visa to go to Russia and had to do this, but there are timelines involved, fees to pay, and forms to fill out for the visa.
When a passport application is filled out there is no countersigned needed by the government. You fill the form out, get a passport photo, and pay the filing fee.
Pay your child support.
If a woman chooses to take her husband's surname (it is not legally required in any US state); she first needs to authorize a name change petition. Submit it to the County Clerk (or Probate, wherever is deemed appropriate in your State). After approval of name change and a court hearing (if needed) you will be granted a Order of Granted Name Change. Then you can proceed to the DMV (to change driver's license), Passport, and social security office. It is also recommended you go to your bank and other institutions where your name should be changed.
