Here's the traditional way of addressing an envelope. Let's pretend Mitch Longley and Dinie Slothouber are married, and you'd like to send them a Holiday card. You would write:
"Mr. and Mrs. Mitch Longley".
The word "Mrs." clarifies that your Holiday card is meant for both Mitch AND Dinie. There is no need to add Dinie's name to your envelope.
No. By convention you list the husbands name first and then the an and and then the wife's name. A parentheses is used for a maiden name. It is not normally used on the outside of an envelope. You assume she knows her maiden name. However, if there are a number of Mary Smiths, you can use it to distinguish which Mary Smith. John and Mary (Harding) Smith. There may be several John and Mary Smiths attending a convention, but only one will be Mary (Harding) Smith married to John Smith.

A pattern envelope is the outside cover of a pattern. It is traditionally a paper envelope, but it can also be a clear plastic bag. The traditional paper envelope is printed with an image of the project to be made with the pattern, along with a complete list of all the materials needed to complete the project. Patterns enclosed in a clear plastic covering will have this same information printed on the pattern cover, which can be read through the plastic.

Before printing the envelope you need to go to the printer's options and select "envelope" from the drop-down media typelist. If you are using Word 2007 envelope printing it will go directly to printing without letting you confirm the printer and media type setting, using the last setting, so make sure to change it while the document is still onscreen, then "close", then do the regular "mailing" stuff. The envelope is way down the list of media type selections after "transparencies". It fixed the problem for me.
