In season 4, after getting paint and feathers dumped on him, Rick goes a gets his fathers gun. He shoots Jimmy and he ends up in a wheelchair. Meanwhile Toby,Emma, and Sean try to escape. Rick stops them... He says a couple things and points the gun at Emma. Sean runs up and tries to get the gun out of Rick. Its unknown who pulled the trigger, but it resulted in Rick dying.

I don't think its highlighted in the actual book - but Jacob does mention how his Mother died - and that was in a car crash. I'm only guessing - but to me it seems that that would be an appropriate answer for Billy being in a wheelchair. Then again, I could be completely wrong. Hope this helped :)
