Assuming we are talking automatic transmissions, you would use park or neutral to start the engine but neither one are gears.
In 2000, the 5.3L only came with an automatic transmission, and the automatics then only had 4 gears.
i think the energy (fuel) is more consumed in automatic shifting of gears whereas the same is done manually.. this could only be the reason..
the only solution that you can affect at home is to check fluid level, and change the fluid and filter this should be done about every 30,000 miles, all other issues need to be dealt with by a reputable transmission specialist.

Physically, Manual Transmission has at least 5 gears including a reverse gear which is 'manually' changed by the driver during the course of the drive. Automatic Transmission however has a Park, neutral, drive 1 and 2, reverse. This is much easier to drive. They can be distinguished by the sound they make. A manual transmission hardly makes noise when gears are being changed. An automatic on the other hand, will make short bust of noises when gears are being changed. This is only valid for high powered cars. Also, if a manual car breaks down, it can be moved. automatic however, cannot be moved if it breaks down which is a big disadvantage.
If it is equipped with a automatic transmission, then it has one. If your 4 runner has a clutch, then you do not have a torque converter. As a rule of thumb, only automatic's have a torque converter.

If it chatters when you start up I would be concerned. If some of the gears do not work, the clutch may be the problem.If the pedal is too low to the floor or gears strip or grind when you shift, it may only need adjusting.Fortunately, compared to automatic transmission, clutches are cheap to fix.
