Coughing up blood in phlegm doesn't definitively indicate COPD, although some COPDers may experience it from time to time. It is actually a symptom of several possible disorders, with the most common being mild bronchial infection (for example - bronchitis, which is a major component of COPD).
Although most cases are mild conditions, it may be a sign of a serious illness (particularly tuberculosis). So if you or somebody you know experiences frequent episodes of coughing up or vomiting up blood, they should contact their physician or health care provider as soon as possible.
You don't taste or smell 'blood', your coughing may irritate your lungs and the irritation may have you taste what you think is blood. As for the smell, you could be smelling the flem. The flem gets that kind of smell after being to much of an irritant to your lungs or throat.
Because something is probably wrong and they should see a doctor.
That is the good question and you probably know the answer. In case of the COPD patient you have less perfusion of the oxygen. The red blood cell production is stimulated by the low concentration of the oxygen.
