whenever you stretch up a size, yes. quite a bit. and they smart for a day or two after that. particularly the first time your stretch them. but it starts hurting less after each time you do it, particularly if you make sure your ears are warm and the blood is circulating well.
for the first time you stretch your ears (at least!) you should definitely get it done at a professional piercing studio. they have graduated needles which stretch your ear much more effectively. Also, most of them won't charge as long as you bring your own gauges or buy them from the studio.
but remember, most importantly, keep your ears clean or they'll get infected! saline (salt water) does the trick!
Well considering you should be doing the stretching correctly there should be no pain involved in stepping up one gauge at a time.

Depends on how sensitive you are, and where you're getting pierced. The lower part of the ear will hurt way less, the upper part is mainly cartilage so you may experience stinging. With "industrials" they pierce two parts of your ear, so double the sting if that's what you plan of getting.
