Yes, she does. She dies saving Ivypool from Hawkfrost and two other Dark Forest warriors. While Ivypool is busy fighting off the two Dark Forest warriors, Hawkfrost kills Hollyleaf.

She could've used that escape to get out, and become a loner. And, the title of the 5th book in the omen of the starts is called the Forgotten warrior. The Erins confirmed that the book would have to do something with Hollyleaf. (It could've been a ghost but I don't know) Therefore, leading to Hollyleaf never really had any last words.... Sorry I had to type all this...
