According to the American Film Institutes Catalogue of Motiton pictures, it seems that "Holy Smoke" may not just be the fictional name of the equine character, but the name of the actual horse itself. The trainer is listed as Jimmy Williams. The film was released in 1966. If this is the case, the horse in question *may* be an Appaloosa born in 1955 named "Holy Smoke" (Biltofts Chico x Christy). The full pedigree in question can be found on the all breed pedigree online site by searching "Holy Smoke Appaloosa". However, I can find no photo of this particular horse with which to confirm.

Emphasyma (in humans) is from second hand cigarette smoke. If a horse has emphasyma it may be from inhaling smoke over a long period of time. But what is likely the case is that the horse has what is called the heaves. An unfortunately incurable problem much like chronic bronchitis. The heaves is caused by lung infections that have been left untreated and that have destroyed the cells in the lungs beyond repair. Try researching the heaves in horses and see where that gets you.
