We do not know. Professor McGonagall's Animagus form is a cat, as is her Patronus (we see that near the end of book 7). However, of the Animagi we know about (James, Wormtail, Sirius, McGonagall, Skeeter), McGonagall is the only one whose Patronus we see. However J.K. Rowling told us that James' patronus is also a Stag.
We do learn that a Patronus can change, as Tonks' is reported to do, as a result of life events, but we are led to believe, by the requirements of registration at the Ministry, that the Animagus form does not.
It can only be a guess, but it seems likely that both the Animagus form and the Patronus form are set by the person's personality at the time of casting the spell, and the Patronus can change as the person changes, but the Animagus spell, that creates the Animagus, is cast only once. So: If Harry, in his third year, had been skillful enough to cast the Animagus spell, we can guess that his Animagus would be a stag; however, after he settled down with Ginny, his Patronus could change to be something more fiery, like a fox or a dragon, but his Animagus would remain unchanged, having a relationship to who he had been rather than who he had become.
Since a patronus is a fictitious thing, you can make up any form you want.
Tom Riddle, aka Lord Voldemort, cannot produce a patronus as he doesn't have any happy memories.
It has never been stated in the books or films what Nevilles patronus is - and J.K. Rowling has never said anything in any of her interviews.

J. K. Rowling does not mention Hargid having a patronus in any of the books. This may be because Hagrid got exspelled in his third year. I do think if Hagid had a patronus it would looks like Fang, his dog. :)
In the Harry Potter series, a Patronus is a charm meant to help defend yourself against dementors. Remus Lupin taught the spell to Harry in his third year. It creates an etherial creature that can deliver messages and chase away bad guys.

Amimagus is the opposite to a wherewolf. A wherewolf transforms into a wolf in the event of a full moon, and the time there isn't any full moon it is human. The amimagus is a wolf which transforms into a human when it's full moon.
Of course it could--Why would it not be able to be something extinct? There's no reasoning in any of the books or interviews with Rowling that would suggest otherwise, and as it is a patronus, not the actual animal, it makes no sense that it could not be such an animal.
