SIM cards do not have to be manufactured by the same company as the phone itself. There are many manufacturers of SIM cards out there.
The PUK code for a phone is not based on the phone itself, it's a safe pin code for the sim-card. Usually the PUK code is written on the paper you got from your phone company if not, you need to call the phone company to get the PUK code

I dont know why but i think you can just buy an iphone off amazon.com or overstock.com and put in your own phone company sim card into it. (EX: like I could put my T-Mobile SIM card into the iPhone.)
