What are the Nursing considerations for digoxin?

1. Assess the patient's response to the digoxin by evaluating relief of symptoms such as dyspnea, orthopnea, crackles, hepatomegaly, and peripheral edema. 2. Monitor the patient for factors that increase the risk of toxicity: a. Decreased potassium level b. Use of meds that enhance the effects of digoxin such as oral antibiotics and cardiac drugs that slow AV conduction and slow heart rate c. Impaired renal function especially in patients 65 years and older 3. Before administering the drug, assess the patient's apical heart rate. If it below 60, the drug should be held. Is digoxin a therapeutic drug?

Digoxin is the best drug for congestive heart failure.Traditionally the Digoxin comes with the packing of 0.25 mg in tablet form. The physicians have been trying to use the drugs from ages since it's discovery. The drug has been giving toxic effects. The physicians tried to give holiday on each Sunday. Then tried to add one more on Saturday, without much benefit. Ultimately the physician stop using the drug. The basic fault lies with the quantity of the drug in the tablet. This much quantity gives toxic effects. Physicians are unable to manage the problem and the drug is defamed. The correct dose should be 0.18 mg per tablet. You can give this much dose with out any toxic effects, that too daily, in most of the patients. Only thing is that the kidney functions should be normal. You may need to give potassium supplement in some, specially when the patient is taking no food for a day or two.
