The Doctor in the future, went back to the Roman time using the transporter that River left behind to give Rory his Sonic Screwdriver. By giving him the sonic screwdriver, he told Rory to go and open the Pandorica with it. He did, and the Doctor found out that he had indeed come from the future, exclaiming "Oh good, I have a future." Rory then, told by the future doctor who wears a Fez and holds a Mop from some musuem, puts the screwdriver into Amy's coat pocket. The Future doctor then finds it in Future amy's pocket.

A Weeping Angel touches her husband Rory, sending him back in time. Because they (The Doctor, Rory, Amy, and River) all saw Rory's tombstone and the dates, The Doctor could not go back in time to save Rory. The Doctor said earlier, in the same story arc, that once it is written down and read, it has to happen. So Rory has to stay in the past and never return. Amy, sadden by this, chooses to be with her husband and allows the same Weeping Angel to touch her and send her into the past. Then the Doctor and River sees Amy's tombstone and now both Amy and Rory are stuck in the past and the Doctor, even with his Tardis, can not go back and get them. That is why and the how of Amy leaving Doctor Who.
