Hundreds, and perhaps thousands, of different individual spacecraft have visited
Earth. As far as anybody knows, every one of them was built on earth and launched
from earth before it landed here. In fact, everyman-made space probe, regardless
of what or where it was ultimately designed to explore, has visited earth at one time
or another.
The Galileo mission and Venus Express have taken photo's and measurements of Earth during flybys and orbital maneuvers.

Depends on whether you say Pluto is a planet or not. If you say it isn't, then there are no planets that haven't been visited by a spacecraft. If you say it is, then there is one that hasn't been visited yet, but one is on the way.
A spacecraft left earth in late 1957 and returned in early 1958. Other than human spacecraft, no alien space craft has ever left or come to visit our planet, that we know of. It is possible some alien space craft may have visited earth millions of years ago. It is highly unlikely any craft has done so within the past few centuries.

It's not clear what you mean by "visited", here. All man-made spacecraft ORIGINATED from Earth. Some of them later RETURNED to Earth as well, and quite a few of them were designed to STUDY Earth (for example, weather satellites).
