You should clean it out if any droppings are in it. Also if it is wet or damp. Other than that you might need to clean it every now and then just to keep it fresh.

Horses need to be exercised every day for 30 minutes or more and they should eat chaff, pony/horse mix and sugar beet, should have fresh clean water available at all times.

Horses should never stand in poo. This causes very serious hoof issues. Daily cleaning is a must for our horses and should be for yours. Missing a day here or there will not hurt the horse, but does set up a pattern of less then great care. Three times a week would be abuse in my stalls.
