  1. Hydrogen and Helium formed in the big bang.
  2. Helium through Iron/Nickel formed by fusion in stars.
  3. All the rest of the elements formed in supernovas by a combination of fusion, neutron capture, and decay. Supernovas produce many elements way into the transuranics, but as they all decayed away long ago, they are not considered naturally occurring.

Heavier elements (up to nickel or iron) form in the hearts of stars in a process called stellar nucleosynthesis.

Elements heavier than iron form in supernova explosions.
