How can it be that the fourth Hokage was dead while the third still alive?

easy in one of the naruto episodes when leaf are looking for a new hokage they said that the former hokage can retire and place someone younger as hokage so if the new one dies the old hokage if still alive takes the place as hokage again in this case the 4th died saving the city so the 3rd had to resume the role us hokage.
