I can think of a few possibilities. One possibility is that he has a convertible. Another possibility is that he has a suitably large sunroof (or moonroof) that can be opened and was standing up in his car with his head outside the car. The other possibility that occurs to me is that he was in the trunk, which would count as being inside the car, with the trunk open.
