you must use a wedge on the outside above the handle to see inside and then use a coat hanger or something long to pull the handle rod

You will see a couple of steel rods attached to the handle, remove these(they just unclip)and move them out of your way. You should be able to see a couple of small nuts holding the handle to the door. Remove these and the handle can be pulled out of the door from the outside.
