Call the Rehab facility and ask the administrator if a sheriff's deputy is capable of serving a patient at that center. If they are not, then you will need to send the papers as a certified letter. They have to sign the letter to accept it, so mailing it as restricted and putting your name on the letter will be the best way to get them to sign it. If it is signed, they are officially served.
I think you can but the rules are different in each state so check what is legal in your state. I know it has to be served within a certain amount of time before the court hearing so it would be good of he/she is well enough to attend. Otherwise it could be postponed and then you just wasted time.

My x was in jail I went and filed for a divorce in February he was served the papers and he kept the papers for a few weeks he sent them back to the lawyer and we were divorced in June. Did take that long, would have been done sooner if he would have sent them back sooner.
