I guess they would have to stay well away from rats and people who already have the disease :)
People could avoid getting the Black Death by staying far away from the infected, keeping clean (taking baths regularly, washing away the fleas) and staying where there is a 'fresh' breeze. You don't want to stay in a place with no moving air, particularly stale air, as you can 'breathe in' the Pneumonic Plague.
