Prior to 1929, the Vatican was just part of the city of Rome, The Lateran Treaty of 1929 established Vatican City as an independent city-state. It is not part of Italy even though it is surrounded completely by Rome,

For many hundreds of years much of central Italy surrounding Rome was called the Papal States and was ruled by the Pope. In the mid 19th century there was an effort made to reunify Italy as a kingdom. The Papal States soon fell and nothing remained of the Papal States. In 1929 Pope Pius XI and Mussolini signed the Lateran Treaty that created Vatican City as an independent city-state totally surrounded by the city of Rome.
