Knit into the next stitch and instead of pulling the old loop off, twist the new loop back on to the first needle just as you would when casting on originally. Do this twice then continue knitting as usual. You will have made a hole in the fabric.
It sounds as if you are going to make a button hole. This is a popular way to do it, so follow closely. You have already bound off two stitches. Now in the next row you simply cast on two loops as if you were starting the project again. The simplest way to do this is to "daisy chain" two stitches on your needle and continue to knit to the end of that row. By the third row your knitting will have the same number of stitches as your base row.
It might be helpful to practice this with a sample piece to see how it works. Cast on six stitches, knit across the row, then k2, bind off 2, k2. Next row k2, cast on2, k2. Next row k6. I find that there are some stitchers who double cast on, which might be your issue. A single cast (1 loop in the same direction as your knit x2) will even your piece.
