You can either right-click on the game's icon on the XMB and click Eject, or tap the Eject button on the front of the PS3. The eject button looks like an arrow with two lines underneath it. This is located in front of the disc slot, immediately adjacent to the PS logo on the front of the machine.

It is most likely the disc error, or a problem with your PS3. Try wiping the disc and restarting your PS3.
