Fish use a system called counter current exchange of water and blood between these two arteries across the gill lamilae (gill sheets), meaning that the blood and water flow in opposite directions across the gill sheet. This maintains the concentration gradient of O2 between blood and water so that more highly oxygenated water is meeting blood with lower oxygen levels. Its quite efficient as well (~90%), which is important since fish live an extremely low O2 environment >2% DO.
