You contact Winn Dixie human resource dept they will give you a form to have your physician fill out then the approve or deny it, if approved and you have the short term disability coverage you get a application from MET life and have it filled out and they send the checks directly to you it is at 60% rate of pay and they mail from Pa. you will not get it on the normal pay day so don't expect it on any certain day we have gotten 3 and none have come on the same day.
