Today, there are three sizes available and the appropriate size is typically determinate on the size of supporter or compression shorts usually it is packaged with. There is the youth size (generally for boys 12 years-old or younger); teen (between 12 and 15); and then adult (15 and older). Of course, there will be gray areas in between the sizing and ages because some guys have a larger (or smaller) overall a body size than others their age.
If really unsure about the right comfortable size, at least pick the appropriate cup/supporter or compression shorts to fit and then purchase the cup separately. If the size purchased is not comfortable enough, go up or down in size.
Importantly though...since guys generally don't like to wear cups, getting the right size (or larger if possible) is crucial so he at least wears one. Nothing is more painful than an injury in this area.
Supporters and cups are not sized the way you might think. It doesn't make a whole lot of sense. The waist size is the ONLY measurement that is ever dealt with on either sports cups or jockstraps. Find your waist size and go shopping. If your "package" or "junk" is smaller or larger than average, you will have to buy a jockstrap with a smaller/larger waist size and find a seamstress who can alter the waistband for you. Lastly, if the cup you buy doesn't fit, the store will not take it back (obviously) and if you need a cup size that is not made by the groin cup manufacturers, you are out of luck. No one makes custom cups because it is too difficult and expensive to build a unique one, and fitting something to fit the genitals is considered extremely weird.
