Hold each paw as you work and spread the toes. Be careful to stay away from the quick, which is easier to cut into than you think. The quick is the end of the toe's blood vessel, so if you cut the quick, the toe will bleed.

Carefully cut the nails, like you would a humans. Be careful to not cut the quick because it does result in extreme pain in the dog.
They sell a medicated powder at most pet shops. You sprinkle it over the place where you cut and it helps stop the bleeding/disinfect the cut.
My dog's toenails are not black. They are white. Not all dogs have black toenails. Not sure what made you think that but..........you are mistaken.

No, nurses are not allowed to cut or clip a patient's toenails. They are however allowed to file them down. Typically a podiatrist will cut them, or a nurse who specializes and is certified in foot care is allowed too.
