When you say leaders, and if you mean kings, queens, etc, they take power by bloodlines, that is, they are appointed the next baron, king, queen, prince in succession, etc by being the next in the bloodline. Your question is vague though, as there are governing leaders in monarchies that are not noble themselves.
Power is transferred in absolute monarchy by the death or overthrowing of the current leader.
He either takes it or inherits it.
Born into power, marries into power, takes power by force.
which one? the current king carl gustaf is a good leader but kings and queens here have limited power. we are a constitutional monarchy, not a full monarchy.
the puritan leader that abolished the monarchy and ruled as a military dictator was Oliver Cromwell.
the usual leader of the English monarchy is a man
The head of state in a constitutional monarchy is the MONARCH, who may take a title of "King", "Queen", "Prince", "Princess", "Sultan", "Emperor", "Caliph", "Maharaja", or "Emir". The head of government, however, depends on the style of government established to curtail the Monarch's power. Usually it will be a PRIME MINISTER or similar person with a different title (e.g. CHANCELLOR) who represents an elected parliament, but it can be a leading oligarch or aristocrat.
