Establish in the first instance what the shortfall is. Identify if that shortfall is yours, someone else's Identify what needs to happen to improve the shortfall set out procedure to improve. i.e. Training for person X on the issue, how the issue occurs, how to recognised the issue and what is done to stop the issue occurring, in this case doing a job/task a specific way and the steps to do that job in that manner. Set in place the training. Set out stages at which the knowledge will be tested and to if its been learned i.e. the person explains back the procedure in a manner that allows you confidence that they can-do the task, or even watch them do the task at the point of checking. All training development processes should have time related stages , therefore x is to be completed by Y date, check to make sure development is working by Z date then the next step to be done and the date for that. Mile stones are vital as is the small stage steps so that everyone know whaere tey should be and at what stage they need to be. Complete the training/development and test it by letting the person do the task fully. Record what has been done, why and with whom and what the required outcome is to be. Get all in the loop to sign off. Set out points in time where the task/process is checked by a competent person to make sure that there is no issues. The process is very "quality management" where you identify the shortfall no matter how small, put in place process/procedure to improve (including communication of the improvement to all involved) allow the process to run, retest the process to make sure its happening if not return to starting point (identify shortfall) and redo. If its working look for further opportunity's to improve and again implement, test improvement, start again. This cycle is used to keep on providing improvements - nothing is ever 100% right so there should be always chances to add small but effective improvements to anything
