ANSWER:

The seven churches of Asia are the seven periods of church history before Jesus returns.

Revelation:1:19-20

19: Write the things which thou hast seen, and the things which are, and the things which shall be hereafter;

20: The mystery of the seven stars which thou sawest in my right hand, and the seven golden candlesticks. The seven stars are the angels of the seven churches: and the seven candlesticks which thou sawest are the seven churches.

Jesus clearly says that John would write the things he had seen(past),and the things that are(present),and the things which shall be(future) right before he goes on to speak of the churches.

We currently live in the last period which is the Laodicean church.

For further study,please read the book(which I have provided a link to)"Daniel and the Revelation"by Uriah Smith.

What different denomination names origionated from each one of the 7 churches in Revelation chapters 1 - 3?

There is no connection between the seven churches of Revelation and contemporary denominations. There are no denominations that trace their lineage to one of the churches mentioned in the apocalypse. The churches in Revelation were gatherings of believers in seven established cities of the time. The very concept of a "denomination" did not arise until many centuries later, and certainly not until after the Reformation which began in the 1500's. If any legally recognized denomination were to use the name of one of the ancient churches in their title, it would be nothing more than a label without substance.

To Whom was the Book of Revelation written for?

The book of revelation is doctrinally written to seven churches that will be on the earth during the time of the great tribulation. The Church of today will not be on the earth during the time that revelation unfolds on the earth. These tribulation churches will be required to believe and express faith in different ways much of the time than the Church of today. That is why much of what Christ says when speaking to these seven churches in revelation is somewhat different than what Paul wrote to the church of today in Romans, Galatians, Ephesians, ect.
