Modern ball point pens have springs and retractable points, but they are basically the same as the original model. A tube of ink feeding a nib, or point, that contains a tiny ball.
There have been a few other changes. There are pens that write under water, or that write upside down. Most pens are gravity fed, so they stop writing unless the point is lower than the ink.

Ballpoint pens generall have several different parts. Ballpoint pens have a main body which holds the ink. The ink is usually in a long tube made of plastic or metal. Ballpoint pens also have clips to attach to shirts, files, or papers. There is also a spring inside the pen that keeps the writing end safe. The writing portion of the pen is usually a small metal or plastic cone with a very small ball bearing inside.
They are both ballpoint pens. The pen introduced in the UK was manufactured by Biro and the popular one sold in the US was (and is) made by Bic. They are very similar. Some people call all ballpoint pens "biros" or "bics".
