If you have lost your confidence because of your horse, it may help to ride a calmer horse of a friends. If that is not availlable then you could try to spend more time with your horse, grooming is good, so you can understand each other more. it may help to have a few riding lessons with an instructor, who may be able to point out if you are going wrong.
When a horse is acting up, you have to show them who is the boss. Most horses will try to test a rider to see how much they can get away with. Do not let them get away with anything. To build your confidence, you just need to keep riding.
Ok well as the saying goes get back on the horse. If riding is something that you are serious about the feeling to ride will overpower your fear of falling off again and if that's not the case and you still feel insecure about riding ride an older or more relaxed horse that is somewhat to easy for you to be riding and eventually you will feel comfortable riding again!

I would leave the place feeling great about myself. The feeling of accomplishing something, of having a partner that is closely connected to you (the horse), and of taking care of a horse, all improve your self esteem.
Find a good riding instructor friend, or parent that will be with you when riding to help you. Find a horse that is at your level, not beyond and not below, take your time it is not a rush, and have fun, do things that are fun for you and fun for your horse. When I started to ride I enjoyed small simple patterns, it is a change and it allows you and your horse to try something new each time. Hope this helps.
