Unfortunately, you cant. You would need to go into your custom teams roster and select a player and click edit player. Then you can change all of his settings such as attributes and all of the appearance related things such as face, skin color, helmet type, age, weight, height, etc. to mold him into the player you want.
Yes, all Madden Games have make your own team option. The only game that you can't is Madden 2004 and down.

I know in the PC version, after you create your team and save the roster then the team automatically shows up when you go to play now. As for other versions I assume you must select load roster/created team and then choose your team, or choose an export option for your team.

when you choose what type of team you are at the bottom it should say "fantasy draft"
Go to the players position when your choosing players for your team, he should be right there at the top depending on the ratings that you gave him.
You cannot make a new team (ex: California Cupcakes) but you can use an existing team and do a fantasy draft (Drafting of existing players) and make a tottaly new team roster
Create a player in Creation zone and put him in any team you want but make sure your team has room on their depth chart
