WikiAnswers' higher Supervisors are the ones who take in the suggestions and add them, regular members do not add categories or sub-categories.
Usually if you add a question to the subcategory of a main category, it automatically removes itself from the main category. You don't *have* to remove questions from main categories if you add them to a subcategory, but doing so would make the site a little more organised and less cluttered.
You can report an incorrect answer at wikianswerscom by reporting the error to the category supervisor.

Yes, in general. A category and a subcategory are practically the same in terms of why we would add one to the site. In one specific instance (adding a top-level category), it is much more difficult to get one added, so adding a subcategory is definitely easier in that case.
