What you do is go to youtube and log in. Then click your youtube channels name in the top right corner, scroll down to video manager and click it. There, it should show you all of the videos that you have made. Under the title, it should say edit with an arrow next to it. Click the arrow and click annotations. Then click "add annotation" and click label. On the bottom, it should say link with a checkmark box. Then add the link!

No, your video must be uploaded onto YouTube before you can add any annotations to a YouTube video. It shouldn't be much of a problem however because adding annotations should hopefully only be a very small part of your video(s) and can also be done fairly quickly and can also be edited.
