You add an affix by going to your horse's page, and pushing the little + button to the right of their name. Then you push Edit Profile and there will be a drop down box where you can pick the affix.
To reserve the affix, you go to your office (under the breeding tab), and click on the tab that says My Affixes.
Remember, if you didn't produce the horse, you can't change it's affix.
