I would address it as follows: First name "nickname" Last name
The proper way address them is Mr. or Mrs Last Name
To address an envelope, write the recipient's name, and, beneath it, address.
On the envelope in the left upper corner you have the return address for ex. the first thing would be your first and last name then below that write your address below that... you put your city, state, and zip code! Then in the center of the envelope you put the person's fist and last name then below it their address then below that again... their city, state, and zip code!

Address it as 'Pastor---(the name of the pastor).
When addressing an envelope to a family, write the family's last name followed by "Family" or "The Family" on the first line. If you know the names of the individuals in the family, you can choose to address it to "Mr. and Mrs. Last Name and Family" or simply "The Last Name Family." Make sure to include the complete address with the correct postal code.
