If Dr, Anderson is particulatily touchy about her social status, AND she is the guest of honor, you may put her name first.
"Your honor"
"Your Honor" is proper.
Your Honor is the most common address. Judge xxx is also acceptable.
You may always address the Jude as Judge for life as a courtesy, but the spouse never did or never will have an official title unless they acquire one on their own merit.
A judge should be addressed as "Your Honor" or "The Honorable Judge __"

depending on the law in your state but if you cannot locate them you can put in for a posting to last known address. It has to be approve by a judge.
Address a judge unknown
The word judge is a common noun. Judge is a proper noun only when it is part of a proper name or title, such as Judge Greg Mathis, actor Judge Reinhold, or the comic book character Judge Dredd.
Consult a lawyer. I'm sure it's possible to appeal the judge's decision.
His honor or Your honor is the proper salutation for a retired judge. The use of the word judge in title and introductions is not used after retirement.
If your spouse is served with divorce papers, that is what matters. The next step is the hearing before the judge. Your spouse may attend or may choose not to attend. It does not matter. What matters is the judge's decree. Many divorces have been granted when the spouse refused to sign.
