"¿Cómo te llamas?" means literally "how do you call yourself", commonly translated as "what is your name". Your response would be to say "Me llamo___", which means "my name is____".

There is another form of this question, "¿Cuál es tu nombre?". This is literally "What is your name". Your reponse would be "Mi nombre es___".

The response you use would be guided by the question. Either form is correct.
