Basically, it is a matter of learning integrity and self-control. It means keeping promises to yourself and others. The exact definition of what it means to be a disciplined person varies. Some would include neatness, most would include punctuality.

As a start, you make up your mind you are going to do something, and you do it. If this is difficult, then you get someone to hold you accountable and make you do whatever. If you want to lose so much weight in 3 months, then you may need a trainer. If you want to learn a new sport and develop some proficiency at it, then you might need a trainer or coach. You just have to really want what you are after, have good willpower, decide you are going to achieve whatever in your life, and surround yourself with people who share your goals or are willing to help you make your goals come true.

Of course, you can join the military or enroll in college, and they should help you develop self-discipline, if that is what you really want in your life. If you don't really want that, then nothing they can do will help you, and you will eventually get kicked out or flunk out.
