Green and bare are both grounds and can be connected. To double check you could use an Ohm meter and make sure the green wire is connected to the cook-top chassis.
Simply run a wire from that outlet to that wall switch. Be sure you use the exact same wire size that you find in that outlet. It will be AWG 12/2 or 14/2. Do not mixes wire sizes. One of the conditions that would cause this is that there is no supply voltage to the top of the meter. Another problem could be, the neutral tickler wire has become disconnected.
