There are a few different things that may be causing the problem.
1) You have a defective mouse - Replace it.
2) The surface you're using is not reflecting the laser properly (glass?) - Use a mousepad
3)The driver files are not working properly - Start-> Control Panel-> Mouse-> Hardware-> Select mouse-> Properties. Then check all of the driver settings. You can also update the driver from here or roll it back to a previous version (if you recently updated and then began to have issues).
