Usually you die when a player or monster kills you.

When you die, you "re-spawn" (re-appear) at your current respawn point; by default, this is near Lumbridge Palace. You lose all your items, except the three most valuable ones. If you are "skulled" (typically if you visited a PvP world during the last half hour or so), you lose all items. That refers to items you carry with you (inventory and worn equipment); items you left in your bank won't be lost upon death.
