The original base is inside the church, to the right, with a gravestone marker. This is what you have to search to find Serena's item.

∙ 12y agogo inside the church at wormwood creek there you will find a giant stone alter to the right of the preacher in a separate room. Click it and you'll find the serene necklace. Hope I helped :)
