After the main quest is complete there are two options.
A. The game is over, you die, it ends permanently.
B. You go to your game console's store and buy a DLC for Fallout 3 called "Broken Steel" which continues the main quest. And when you beat Broken Steel the game keeps going. It should cost about $10.
After you finish the main quest the game ends. But if you own it on Xbox 360 and have Xbox LIVE, Broken Steel will extend the main quest and will raise the level cap from 20 to 30.
