After the Ostagar scene near the beginning, you will arrive at a village called Lothering. Leliana can be found at the local tavern.

no, but you meet some familiar faces: Flemeth, Alistair, Leliana, Bodahn Feddic and Sandal, Anders from Origins: Awakening, Bann Teegan and Lady Isolde, (Bann Teegan and Alistair in the Viscount Keep Lady Isolde and Bann teegan together in Chateau Haine.) I think that's all of the characters.
Yes, but a bug can sometimes stop you getting it. If you raise her approval too high, too fast, then you may not get the dialogue options to unlock her personal quest.
It only depends on your choices. If you have played Awakening-the expansion pack toOrigins you will meet Anders and he will be a companion/love interest. Alistair, Leliana and Isabela are also returning as well as Zevran and Bann Teagan, but this all depends on choices. Good luck!
